As we remember the foods that raised us, brought us joy and connection, we will cement these memories and recipes into a cookbook to be passed down from generation to generation.

Feel free to bring your own recipes or photo memories. Cookbook making supplies will be provided.

To participate please read our Liability Waiver and Media Release below by registering for this event, you will acknowledge you have read and agreed with our general liability waiver.

Liability Waiver

By completing the reservation, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Dances Unlimited. I/we agree to release and hold harmless Dancers Unlimited including its teachers, dancers, staff members, volunteers and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold Dancers Unlimited liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the event and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Dancers Unlimited.

I understand that Dancers Unlimited is a licensed and insured business. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to the Owner, Director, Instructor or Staff member as soon as possible.

Media Release

I authorize Dancers Unlimited (the "Organization") and its partners, agents, representatives, assigns, successors in interest and licensees, to photograph, audiotape, and/or videotape me and grant the Organization and its partners the irrevocable right to use my photograph, audio recording, video recording, or any reproduction or modification thereof (the “Photograph,” “Audio,” and/or “Video”), in any manner or medium throughout the world an unlimited number of times in perpetuity in advertising, trade, promotion, exhibition, or any other lawful purpose. I understand that I will not receive any monetary compensation for the permissions I am granting herein. I hereby waive any right of inspection of approval of the uses to which the Organization may put the Photograph, Audio, and/or Video. I acknowledge the Organization will rely on this permission and hereby release and discharge the Organization from any and all claims and demands arising out of or in connection with the Photograph or the exercise of the permissions granted here, including any or all claims for libel, invasion of privacy, or emotional distress. I understand that I cannot withdraw my consent after I sign this form and that this consent and release is binding on me and my heirs, legal representatives and assigns.
